Fine Print and Strict Referral Policies:
- The Client You Refer Must Specify BEFORE or AT the Initial Consultation or Appointment that they were Referred by You and Provide Your First and Last name.
- Consequently, your name will go on a “Referral List” pending the client’s service and payment in full.
- Once the referred client receives the service and is paid in full, you will be contacted by Eve via telephone and asked your choice of the incentive reward options above.
- Monetary Incentive rewards will be placed on your account as “Bonus Bucks” for use at your leisure and will never expire.
- Tangible incentives will be prepared for your pick up at EEA at you earliest convenience.
- Only one (1) Referral Incentive will be honored per Referred Client. No duplicate client referrals.
- You are encouraged to refer as many clients as you like and receive a reward incentive for each and every client that meets the referral policy requirements as listed here.
- Monetary referral reward incentives in the form of “Bonus Bucks” can be used and will not affect any quoted or coupon discounts that you may have when you choose to use the credit.
- Referral reward incentives in the form of “Bonus Bucks” can be used on any EEA Service, Tanning, Tanning Products or Jewelry, the same as cash.
- Coupon/Discount policies as posted on the EEA website, are still and will always be in effect for all other quotes or services.
- Incentives cannot be exchanged for cash, refunds, or prior purchases and have no cash value except as depicted above.

A $50 deposit is required to schedule a procedure & will be credited to your balance provided the scheduled appointment is kept or cancelled/rescheduled within 48 hours.
Your time with me is very important. Therefore, I charge a $50 fee for missed appointments, including complimentary touch-ups and prepaid sessions. PLEASE CALL within 48-houirs of your appointment if you must cancel or re-schedule or this fee will apply. Coupons and Discount Policy
Permanent Cosmetics, Collagen Induction, Tattoo Removal, Scar/Skin Needling Sessions and Package Prices are determined based on the following criteria in order for the business to be profitable and offer affordable services. Please do not expect or ask me to circumvent these policies for any reason.
- Please Realize and Understand that at times, Coupons or Discount Offers are publicized via Multiple Different Venues and the Following Restrictions Always Apply.
- Coupons/Discounts CAN NOT be used Multiple Times or Combined with other Discounts Regardless of the Different Venue they came from.
- Coupons/Discounts on Multiples of the Same Procedure CAN NOT be honored although I am Happy to Honor a Coupon/Discount offered on a (1) One Time Basis for Each DIFFERENT Procedure and on Collagen Induction Therapy (4) or (6) Packs where applicable.
- Coupons/Discounts ARE NOT Valid on Permanent Cosmetic Touch Ups. A Discount is Built into the Procedure Quote. Additional Discount/Coupons CAN NOT Be Piggy Backed.
- Coupons/Discounts ARE NOT Valid on Pre-Quoted Multi-Procedure Packages such as Larger Job Tattoo Removal Sessions, Scar Reduction/Softening and Skin Needling Sessions that are Quoted in Advance. A Discount is Built into the Multi-Procedure Quote. Additional Discount/Coupons CAN NOT Be Piggy Backed.
Payment Methods and Refund Policy
Payment in full must be made when services are rendered. We accept cash, personal checks, and major credit cards (Visa, MasterCard, and Discover). There is a $35 handling fee for any returned check. Gift certificates are available for purchase but cannot be exchanged for cash. The results of any cosmetic procedure, including permanent cosmetics, cannot be guaranteed. As such, whether or not a patient is satisfied with the results, there will be no refunds.
Visitors and Children
CDC-OSHA guidelines do not allow observers in the procedure room. Family or friends can remain in the waiting room if desired. They are also welcome to participate during makeup designing. Wisconsin law prohibits the tattooing of persons under age 18. The entire office, including waiting room, is not equipped for children or babies. Child care arrangements must be made prior to all appointments.
